Name changes

When an individual’s name changes as a result of marriage, divorce, etc., please provide a copy of the Marriage Certificate, Divorce Decree reinstating a maiden name, or other official document, such as a driver’s license, as evidence of the change.

When a company or corporation changes its name, please provide a copy of the Certificate of Name Change and the new tax ID number.

Along with a letter of instruction, mail these items to:
