Upon the death of an owner, the following documents are required:

1. 1.   A certified copy of the decedent’s death certificate.
   

2. 2.   A copy  of the Deed showing the Joint Tenancy.
   

3. 3.   The name and address of the successor in interest.
   

Along with a letter of instruction, mail these items to:
